Welcome to the pantry. Our shelves are full and we are looking forward to sharing some of that food with you. We need for you to fill out information about your family the first time you come only. That will be entered in our computer so we can serve you better.

There are 2 forms that you can print, fill out and bring with you on your first visit to the pantry. It will save time while at the pantry. If you have any questions on the forms, please leave that area empty and we can answer the question when you come. If you would rather fill out the complete form at the pantry, we have them available for you to fill out before you shop.

Form 1 – titled Head of the Household; this form should be filled out by the person that will be doing the shopping for your family. There are 3 pages for this person.

Form- 2- titled Household Member; this is a one page form that you will complete for each member of your family that you will be shopping for. (Do not fill out this page for the Head of Household)

When you come to the pantry, we will take your completed forms and then issue you a card that has your number and ask that you present that card any future time you come to the pantry which will speed up the sign in process.
